lava: Include metadata about the submitting source

Add a source metadata object to encode where the LAVA job has been submitted from and how it should be handled.

One of the goals here is to avoid creating Phabricator bug tasks for failures happening on wip/* branches, see also infrastructure/qa-report-app!83 (merged)
