Review of concept documents

This MR contains a review of the concept documents and the remaining documents in designs. These have been classified as "Requires Update", removed as obsolete or left unmarked if appear to be in a good state. The design documents have been moved to the correct location (mainly concepts) and that legacy section removed.

A few minor clean-ups are also included to remove warnings from the build that made it easier to find errors during this work.
