Rename and reorganise workflow and repository documentation

The information about the workflows and repository structure is all
jumbled up. Rename and sort the documentation into high-level
information, workflows and repository layout.

Shift more detailed documentation found in this document into more suitable
locations, rewording/adapting as required.
